Hehe, this had "Wolf's Rain" written all over it for me. ^^;
But, seriously, in a metaphorical sense, a transformation [in this case one of a man(?) into a wolf] usually means surrending to one's true identity (or part of one's true identity as you noted on the 6th stanza). I think this man was trying to escape social conformation and the ruling sructure. The blizzard probably represents how difficult it can be to flee from that, the obstacles he will encounter... the obstacles can be a lot of things, oneself, peers, media, etc...
But it's weird because the poem itself is quite typical, and that contrasts with the message... as does the use of a wolf. I mean wolves usually travel in packs where a hierarchy is established, one that must be respected, so wouldn't that be going back to what he wants to escape? Sure there are 'lone wolves', but even they want to belong, and even search to belong in a pack...

     "As my footprints turn into pawprints out here in the snow/the wolf inside will show me which way to go" -- fuckin brilliant, man! That last stanza really left a creepy vibe, it made the hairs of the back of my neck stand up.
I just loved this verse.
